ADMISSION REQUIREMENTS FOR NEW STUDENTS

 

1. Original FORM 138
2. Good Moral testimony by the principal
3. Four (4) copies of latest picture (size 1 ½ by 1 ½ )
4. Birth Certificate (NSO copy) with registry number (photocopy)

 

 

Additional Admission Requirements

 

1. Applicants must pass the qualifying examination of the university with equivalent rating of 80%.
2. In order to cope with the demands of the nursing profession, the applicant shall be evaluated by the program head with respect to relevant factors such as physical characteristics, previous academic experiences, motivation and others.

  

 

GUIDELINES FOR TRANSFEREES

 

1.      Any course not specifically related to Health Allied.

2.      Requirements for Admission for nursing

3.      No subject to be credited(Professional only)

4.      Should be from a recognized school – CHED Grant Permit

1.      General Average Grade of 80%

2.      Good Moral Character –inter-college
